Unzip this file to \Valve\Steam\SteamApps\my-email@mail.com\half-life 2 deathmatch\hl2mp\ and keep the foldernames. This way
you'll extract the map and the textures to the right directory

Do NOT distribute these files seperately or the level won't work correctly.
